Síndrome antifosfolípido

El síndrome antifosfolípido (SAF) es un trastorno autoinmune caracterizado por numerosas manifestaciones clínicas, hematológicas y serológicas posibles. Entre ellas destacan la trombosis venosa o arterial, los abortos espontáneos (recurrentes) o los nacimientos prematuros consecuencia de la (pre)eclampsia o de insuficiencia placentaria.

En la mayoría de pacientes aquejados de síndrome antifosfolípido, pueden detectarse determinados anticuerpos llamados antifosfolípidos (por ejemplo anticoagulante lúpico (AL), anticuerpos anticardiolipina (ACL), anticuerpos anti ß2 glucoproteína-I o anticuerpos antifosfatidilserina).

Los anticuerpos antifosfolípidos probablemente induzcan trombosis a través de la activación de la coagulación y/o mediante factores inhibidores que dificultan la coagulación. Los anticuerpos antifosfolípidos (AAF) se manifiestan en el 90% de las trombosis y muestran una mayor frecuencia en otros familiares del paciente con predisposición genética. Las mujeres representan el 80% de los casos.

Los AAF son anticuerpos pertenecientes a la familia de isótopos de IgG y IgM y también, aunque rara vez, IgA, específicos para la coagulación de fosfolípidos activos (fosfatidilinositol, fosfatidilserina, fosfatidiletanolamina y cardiolipina). Los AAF no pueden unirse directamente a estos fosfolípidos, sino que deben hacerlo a las proteínas de plasma, que son complejos con fosfolípidos. Entre las proteínas de plasma destaca la ß2-glucoproteína I.
